Clinical documentation comparison

DAP vs SOAP Notes

DAP and SOAP are both structured documentation formats, but they organize session information differently. This guide compares the two formats and explains the practical differences.

The main difference:

DAP uses three sections — Data, Assessment, and Plan — while SOAP uses four — Subjective, Objective, Assessment, and Plan. SOAP separates client-reported information from clinician observations, while DAP combines much of that information into one Data section.

What is a DAP note?

DAP stands for Data, Assessment, and Plan. The Data section generally contains clinically relevant session content, including client reports, observations, interventions, and other important information from the encounter.

DAP structure

  • Data: session information and clinically relevant details
  • Assessment: clinical interpretation and progress
  • Plan: next steps in treatment

What is a SOAP note?

SOAP stands for Subjective, Objective, Assessment, and Plan. Unlike DAP, SOAP separates information reported by the client from information observed by the clinician.

SOAP structure

  • Subjective: information reported by the client
  • Objective: clinician observations and observable information
  • Assessment: clinical interpretation and progress
  • Plan: next steps in treatment

Clinician observations Usually included in Data Placed in Objective

Overall structure More consolidated More separated

When might DAP be useful?

DAP can be useful when a clinician prefers a more consolidated structure. Because client statements, observations, interventions, and other session information can be grouped into the Data section, the note may feel more streamlined.

The format may also work well in settings where the documentation workflow already separates clinical interpretation and future planning from the descriptive session content.

When might SOAP be useful?

SOAP can be useful when it is important to clearly distinguish client-reported information from clinician observations.

The additional separation may help create a clearer record of what the client said versus what the clinician directly observed during the encounter.

Which is better: DAP or SOAP?

Neither format is universally better. The appropriate structure depends on the clinician's workflow, organization, payer requirements, profession, jurisdiction, and the type of services being documented.

Some clinicians prefer DAP because it is more compact. Others prefer SOAP because the Subjective and Objective sections create a clearer separation between reported and observed information.

Example of the same session in DAP and SOAP format

Imagine a client reports increased anxiety about work, difficulty sleeping, and repeated email checking at night. The clinician observes that the client is engaged and practices a grounding technique during the session.

DAP example

Data: Client reported increased work-related anxiety and difficulty sleeping due to repetitive worry. Client described checking email repeatedly before bed. Therapist reviewed the connection between rumination and sleep disruption and practiced a grounding technique. Client was engaged and receptive.

Assessment: Anxiety remains elevated, particularly during evening hours. Client demonstrates improved awareness of behaviors that may reinforce worry and appears receptive to behavioral strategies.

Plan: Client will practice grounding during anxiety spikes and limit work-related email checking before bedtime. Review adherence next session.

SOAP example

Subjective: Client reported increased work-related anxiety, difficulty sleeping, and repeated email checking before bed.

Objective: Client was attentive and engaged and participated in a grounding exercise during the session.

Assessment: Anxiety remains elevated with increased awareness of behaviors contributing to nighttime rumination.

Plan: Continue grounding practice and reduce email checking before bedtime. Review progress next session.

Can AI generate both DAP and SOAP notes?

AI-assisted documentation tools can help organize the same underlying session information into different structured note formats.

ClariteNote supports both DAP and SOAP documentation, along with BIRP, progress notes, and other clinical documentation workflows. Generated documentation should still be reviewed and corrected by the clinician.

Use the note format that fits your workflow.

ClariteNote helps therapists turn recorded, uploaded, or typed session content into structured DAP, SOAP, BIRP, and progress note drafts for clinician review.
